Luna Vista Home Healthcare is a Medicare and Medicaid certified home health provider as well as an Accreditation Commission for Health Care (ACHC) accredited agency specializing in at-home nursing and rehabilitation services necessary for healthy and independent living. Our agency is devoted to helping our patients achieve and maintain the best possible quality of life.

The administrative and clinical staff at Luna Vista Home Healthcare & Hospice embodies compassion, heart, patience, empathy, and competence to meet our patient’s needs. Our clinical team works closely with the patient and their physicians to assure proper delivery of care. Our goal is to assist our patients with recovery and rehabilitation; educating skills that encourage helpful lifelong health and wellness behavior.

Luna Vista Home Healthcare recognizes that each patient is unique in the level of independence he or she wishes to preserve as well as the assistance required at home to stabilize their health. Several patients are experiencing difficulties in managing chronic illnesses while others struggle with simple day-to-day tasks.

Our job at Luna Vista Home Healthcare & Hospice is to deliver our patients the physician-directed care that is most appropriate for their conditions. Treatment is determined through collaborating with the patient’s physicians and the Luna Vista Home Healthcare clinical staff. This strategic delivery of care ensures that our patients and their loved ones feel confident that the plan of care provided in their home will effectively address their health needs.

Quality Assurance and Performance Improvement

Healthcare quality is our overarching goal at Luna Vista. The goal of our Agency is to continuously improve the quality of services rendered. Our Quality Assessment and Performance Improvement (QAPI) Committee is responsible for monitoring quality measures including but not limited to: staff performance assessments, compliance with clinical practice standards, improvement in patient mobility & independence with ADLs, wound healing, patient falls, and measures to support infection control functions. We strive to improve the overall quality of life and quality of care and services delivered to our patients.

About The Owner

Luna Vista Home Healthcare & Hospice was established in 2014 with the mission of improving and restoring patients’ lives through quality patient care. Owner Jamie Gonzales founded Luna Vista Home Healthcare after desiring an opportunity to use her medical background to not only improve the health and well-being of the communities she grew up in and served as an intensive care unit nurse but to also embrace the spirit of entrepreneurship and small business ownership that exemplifies the quintessential American Dream.

We have grown from a one-woman operation to a 60-plus staff who services hundreds of community members every year!
